Hi Prasad,
As you are using a customized Z report for required output and on print preview you are able to get the output except user name .So here i would suggest you to consult with ABAP team or developer.
For cross ref try to take printout of any other Z customized report apart from ZWM02 .
This issue not related with any printer settings issue seems with coding .
